Fat transfer to the buttocks for fuller, rounder, feminine curves
A fuller, rounder buttock profile is one of the hallmarks of a feminine silhouette — and one of the hardest proportions to achieve through hormone therapy or exercise alone. A Brazilian butt lift uses your own fat, harvested from areas with masculine-patterned deposits, and transfers it to the buttocks to create the volume and projection that bring your lower body into feminine proportion.
A Brazilian butt lift (BBL) is a body contouring procedure that enhances the size, shape, and projection of the buttocks using autologous fat transfer — fat harvested from your own body via liposuction. For gender-affirming purposes, the BBL serves a dual role: it removes fat from areas with typically masculine deposits (such as the waist, flanks, abdomen, and back) while simultaneously building volume in the buttocks to create a rounder, more feminine rear profile. This redistribution of fat fundamentally reshapes the torso-to-hip ratio.
The procedure has become one of the most sought-after body contouring surgeries worldwide and, when performed by experienced surgeons using current safety protocols, carries a well-established risk profile. For transfeminine patients, the BBL offers the distinct advantage of feminizing two body regions in a single operation — slimming the waist and building the curves — without the need for synthetic implants.
A BBL involves two integrated surgical stages — liposuction and fat grafting — performed under the same anaesthetic. Body proportions, fat distribution, and skin quality are assessed to plan a contouring strategy that maximises the feminine result. The techniques used for fat harvesting and injection have a direct impact on fat cell survival and the longevity of your results.
The two key components are:
Fat is harvested from donor areas using either VASER (ultrasound-assisted) or power-assisted liposuction. VASER uses ultrasonic energy to emulsify fat while preserving the surrounding connective tissue, resulting in smoother contours at the donor site. Power-assisted liposuction uses a vibrating cannula to loosen fat gently. Both methods prioritise fat cell viability so that the highest possible percentage survives after transfer.
The purified fat is re-injected into the buttocks using small-calibre cannulas in a micro-droplet technique. Fat is distributed across multiple tissue planes — subcutaneous and intramuscular — in tiny aliquots to ensure each droplet has adequate contact with blood-supplying tissue. This approach maximises graft survival and creates smooth, even volume without lumps or irregularities. Injection is performed exclusively above the gluteal muscle fascia or within the muscle itself, in accordance with current safety guidelines.
Moderate soreness and swelling at both donor and buttock sites are typical after a BBL, controlled with prescribed pain medication. You will wear a compression garment on the donor areas. Avoid sitting directly on the buttocks, use a BBL cushion or lie on your side and stomach. Light walking is encouraged.
Bruising begins to resolve and swelling gradually decreases. The buttocks will look larger than their settled size because of swelling. Continue using the BBL cushion for sitting and avoid driving. Attend scheduled follow-ups for wound checks and compression garment assessment.
Comfort improves and you can begin sitting for short periods with a cushion. Light daily activities and gentle walking increase. Avoid strenuous exercise and prolonged pressure on the buttocks. The body contour becomes more visible as swelling subsides.
Surviving fat cells, typically 60–80% of the transferred volume, are now permanently established. Buttock shape and projection reach their settled state by three to six months. Full exercise, including lower-body strength training, can restart after surgical clearance. Maintaining a stable weight protects your results.
The Brazilian butt lift is a widely performed procedure that has undergone significant safety improvements in recent years. Current protocols — including above-fascia and intramuscular injection techniques — have substantially reduced serious complications. As with any surgery, understanding the risks helps you make an informed decision.
Safety is the foremost priority in BBL surgery. Our partner surgeons follow the latest multi-society safety guidelines, including ultrasound-guided injection and above-fascia technique where appropriate. A thorough pre-operative assessment and conservative volume planning ensure the procedure is performed within safe limits for your anatomy.
